Sliding-Fee Scale Financial Assistance Program
CHC offers a Sliding-Fee Scale to help income-eligible patients pay for their care. If you are low-income, don’t have health/dental insurance or have high deductibles, this program can help. For patients enrolled in Medicare, the Sliding-Fee Scale can be applied to Medicare co-insurance. To enroll, bring in one form of government issued identification and two forms of proof of income; pay stubs or your latest tax return. Patient Support staff will guide you through the application or fill it out and mail it. The application is available in seven additional languages, which can be found here. Vermont Health Connect Enrollment
We offer certified Vermont Health Connect navigators to help you enroll in MVP Health Care, Blue Cross Blue Shield of Vermont or Medicaid and explain the plans. We’re here to help patients with insurance forms, navigate the website and connect you to benefits. For Medicare enrollment, we refer to AgeWell’s State Health Insurance Assistance Program. Other services our Patient Support Services team can help connect you to include: You First, 3SquaresVT, Health Assistance Program (HAP), eyeglasses assistance, patient financial assistance through UVM and more.
Patient Support staff will connect you to the Community Health Pharmacy (CHP). CHP gives you great prices on your prescriptions and offers a handy mail order service. We’ll also work with you to find the best price on your prescription from local pharmacies.
