- View and print medical chart information including problems, history, allergies, immunizations
- View all lab results once available
- Request medication refills
- Message your care team
- See upcoming appointments and request future ones
- Complete forms prior to scheduled visits
- Receive and send messages and patient education

Start using MyChart today!
You can use MyChart on a computer, phone, or tablet. You can also download the MyChart app. You may have already been sent a sign-up link via email or SMS, or you can sign up directly by visiting www.chcb.org/MyChart, clicking “Sign Up,” and then “sign up with your information.”
