As an integral part of the health care management at CHC, the mental health counseling staff is here to help patients of all ages with a broad range of mental health conditions. We employ Licensed Clinical Social Workers who provide therapy and consultation generated by internal referral from our medical staff. Our mental health clinicians also work collaboratively with our medical providers and nursing staff to support patients who are overcoming addictive illness, including a Medication Assisted Treatment Program for opiate addiction. Mental Health Consultants
The mental health staff is available to the medical providers and patients for consultation during the medical visit. A Clinical Social Worker can help assess your mental health condition or addictive illness. We can also provide education and self-management skills training to manage a mental health condition or to alleviate stress.
Medical Case Management
Social Work Case Managers are also available to help with a variety of social services, such as housing assistance, applying for benefits, or help with care coordination.
How Do I Get an Appointment?
- New patient to CHC? If you have not established care yet with CHC, please call to schedule an appointment with a primary care physician. Counseling services are made through internal referral only when requested by your primary care physician.
- Existing CHC patient? If you currently have a primary care physician at CHC, make an appointment with them to discuss your concerns and goals for counseling support.
