CHC offers Suboxone Treatment for opiate addiction. This medication-assisted therapy (MAT) uses buprenorphine (Suboxone) together with education, Mental Health/Counseling and other support needed to manage addiction for a lifetime. Medication-assisted treatment for opiate addiction is much like treatment for other chronic illnesses.
CHC is a part of Vermont’s Care Alliance which delivers ongoing care services for opiate addiction in a model called the “Hub and Spoke.” CHC provides Office-Based Opiate Treatment and is known as a “Spoke” within this model. Hub and Spoke services ensure that each patient undergoing treatment has a medical home, a medical provider, a pharmacy home, and access to nurses and clinicians. At CHC, our MAT team includes a Nurse and Mental Health Clinical Care Coordinator to provide support to patients in their recovery, and is a free service to Suboxone patients. We help lift barriers to care and provide the support patients with complex addictions need to be successful in their recovery.
If you are a CHC patient and would like more information about how to access Suboxone Treatment, please speak to your physician. Others may call Howard Center’s Safe Recovery line at (802) 488-6067 or visit their MAT information page. CHCB’s work with Vermont’s Care Alliance helps with coordination of care between multiple organizations and requires a Consent to Disclose Health Information.
