Community Health Centers (CHC) Serving Chittenden and Grand Isle counties
  • CHC celebrated a year of service to our community at our Annual Meeting on October 29 at 617 Riverside Avenue, where Vermont’s Chief Health Care Advocate, Mike Fisher, shared his perspective on the state’s health care landscape. Our Board of Directors also elected new officers, honored longtime members completing their terms, and welcomed our new CEO, Greg Wilmot.

As a Federally Qualified Health Center, CHC has a patient-majority board: more than half of our board members are active patients who live and work in our community. Their experience helps us understand and respond to the needs of the people we serve.

Our FY 2026 Annual Impact Report is now available at chcb.org/about/publications.
  • Open Enrollment for 2027 health plans is November 1 through December 28, 2026.

You can sign up for a new plan or change your current plan.

- Enroll by December 15 for coverage starting January 1, 2027.
- Enroll December 16 through December 28 for coverage starting February 1, 2027.

To renew or change your plan, call Vermont Health Connect at 1-855-899-9600. For help, call CHC Patient Support Services at 802-264-8124.
  • Today is Wear Pink Day for Breast Cancer Awareness Month. Our staff wore pink to help spread awareness and show support for people affected by breast cancer. 🩷

Knowing your risk and getting screened can make a difference. Talk with your health care provider about when breast cancer screening may be right for you.

Wear pink and help spread awareness!
