CHC is moving from InteliChart to MyChart, a new patient portal.

What is MyChart?
MyChart is a secure website and app that helps you connect with your CHC care team and manage your health information. If you already use MyChart with another health care organization, you do not need a second account. You can add CHC and switch between your connected organizations in the MyChart app.
Plan ahead before we close
Medication refills
You cannot send a new medication refill request on August 8 or August 10. Please send refill requests before the closure.
Phone messages
You cannot leave a voicemail while CHC is closed. Please call before the closure if you have a routine question or request.
Care while CHC is closed
For an urgent health need, call your CHC office. The answering service will guide you to reach the on-call nurse or provider.
You can still pick up medication that is ready at the CHC Riverside pharmacy or the CHC Good Health pharmacy. New prescriptions and refills will not be processed on August 8 or August 10. Please plan ahead.
If you have a medical emergency, call 911.
What is changing with InteliChart?
Friday, August 7
InteliChart scheduling and appointment requests close at the end of the day. If you have a pending appointment request, our team will follow up with you by phone starting August 10.
Monday, August 10
InteliChart appointment reminders, medication refill requests, and portal messaging close at the end of the day. Any pending refill requests or messages will be handled starting Tuesday, August 11. For time-sensitive items, we may call you.
Tuesday, August 11
InteliChart becomes read-only. You can still view your records, but you can no longer send messages or requests. Use MyChart for all new portal messages and requests starting August 11.
You can view your InteliChart records through September 11. After that date, InteliChart will no longer be available.
Start using MyChart on August 11
MyChart goes live on Tuesday, August 11. You can use MyChart on a computer, phone, or tablet. You can also download the MyChart app.

How to sign up
Starting August 11, you will get a text message with a personal MyChart sign-up link if we have your mobile phone number and you visited CHC in the past year. After MyChart is available, you can also sign up using the link that will be sent to your email or by coming back to chcb.org.

Telehealth visits
Starting August 11, CHC telehealth visits will use Zoom through Epic instead of the old system.
If you have a MyChart account, check it before your visit for information about joining. If you have not signed up for MyChart, our staff can send your visit link by text message or email using the contact information in your chart. You do not need to download the Zoom app. You can join from your phone or a web browser.
