David S. Adams, MD received his Bachelor of Arts from Williams College in Williamstown, Massachusetts. He attended the University of Colorado School of Medicine in Denver for medical school and then did his residency training in Family Medicine at Contra Costa Regional Medical Center in the San Francisco Bay Area. Dr. Adams' professional interests include international health and providing full-spectrum medical care to entire families. He speaks Spanish fluently. While not at work, he loves to spend as much time as possible outdoors and traveling with his wife.
John Brooklyn, MD graduated from the University of Vermont with a BS in education in 1979. He received his medical degree from Brown University in 1989 and did his residency at the University of Vermont/Fletcher Allen Health Care Family Practice Residency. Prior to becoming a doctor, Dr. Brooklyn was a ski instructor, elementary school teacher, chef and purchasing agent. Dr. Brooklyn is board certified in addiction medicine and works on treatment projects for drug addicted pregnant women and opiate addicted prisoners. John is also a national mentor for physicians who prescribe Buprenorphine and substance abuse trainer for medical residents. He has an interest in holistic health and outside his busy medical practice, John can be found maple sugaring, raising small animals, swimming, cycling, doing yoga, watching the Red Sox, spending time with his family and speaking French.

Tara Chasnoff, NP was born and raised in Brooklyn NY. She received a Bachelors of Arts in Psychology from SUNY Binghamton. After graduation, she worked at the NYC DOH as a research assistant. Wanting to focus on patient-oriented work, she earned her nursing degree at the University of Massachusetts-Amherst. She then worked as a nurse at NYU hospital and NYU student health center while obtaining her nurse practitioner degree at NYU. After leaving New York she moved to Vermont and worked initially as a nurse practitioner at Fletcher Allen's cardiology practice, and then in two different primary care practices. Her professional interests include cardiovascular health, women's health, mental health, and college health. Outside of work, Tara enjoys dancing, yoga, spending time with her family and friends, and the peace, quiet and beauty of Vermont.
Patti Fisher, MD graduated from Saint Mary's College and the University of Texas at Houston Medical School. She completed her residency at Forbes Family Practice in Pittsburgh, PA. Before coming to CHCB she worked in a private practice. Dr. Fisher's professional interests are in women's health including prenatal care and deliveries, comprehensive health care for the entire family, homeless health care, substance abuse and mental health treatment, and tropical medicine. When Dr. Fisher is not at work she can be found with her wonderfully supportive husband and three children. She enjoys running and biking in the Vermont outdoors. Dr. Fisher is also involved in working at a rural health clinic in Haiti.

Kerry Goulette, MHP, PA-C graduated from College of Holy Cross and received her Physician Assistant certificate and Master of Health Professions from Northeastern University. Before her arrival at the Health Center in 2006, Kerry worked for Lynn Community Health Center and Boston Medical Center in Massachusetts. Her professional interests include adult medicine, heart health and diabetes. Kerry is currently focusing her practice on the adult homeless population in our ocmmunity. Kerry speaks Spanish and loves to run and ski.

Annika Hawkins-Hilke, FNP, MPH was raised in Strafford, Vermont. She received her BA from Wesleyan University and her MSN/MPH from Johns Hopkins University as a Pediatric Nurse Practitioner with an emphasis on community and global health. Annika later returned to the University of Vermont to study adult health, and now works with the health needs of all ages. Her professional interests include health care for at risk youth, homeless health care, and global health. Outside of her work, Annika enjoys travel, backpacking, yoga and cooking.
Rachel Inker, MD Graduated from Harvard University AB Cum Laude and University of Massachusetts Medical School. Dr. Inker's professional interests are as follows: women's health, addiction medicine, transgender medicine and mental health.

Sherry Larose, PA-C was born and raised in Franklin County, Vermont. She received her Bachelor of Arts in Biology from Clark University in 2005 and her Masters in Physician Assistant Studies at the Massachusetts College of Pharmacy and Health Sciences in 2009. Before PA school, Sherry worked as a Histology Supervisor at St. Elizabeth's Medical Center and as an Emergency Department Technician at Beth Israel Deaconess Medical Center in Boston. Her professional interests include all aspects of family medicine especially adolescent medicine, mental health, and health education and prevention. Sherry enjoys traveling, cycling, hiking, snowshoeing, and spending time with her family, friends and two cats.
Lianna Percy, PA-C is a 7th generation Vermonter who grew up in Burlington. She graduated from UVM and completed a second B.S. in nutrition at Hood College in Frederick, Maryland. She has worked as an Oncology Nutritionist for the National Cancer Institute at NIH and later at the Lombardi Cancer Center - Georgetown University. After completing her Physician Assistant program at George Washington University in 2006, she worked in adult primary care in the Washington, DC area. Lianna is thrilled to be back in Vermont and is looking forward to working in her home community at CHCB. When she's not working, Lianna likes to spend time with her family, biking, hiking, and reading.
Margaret Russell, PA-C graduated from Colorado College and received her Physician Assistant and Pediatric Intensive Program certification from the University of Colorado. Before she joined the Health Center in 1997, Margaret worked for Helios Health Center in Boulder, Colorado. Her professional interests include pediatrics, developmental and holistic medicine. Outside of her work, Margaret enjoys all outdoors activities, including gardening, hiking and biking.
Andrew Saal, MD, FAAFP received his undergraduate degree from the University of Notre Dame and his M.D. from the University of Cincinnati. He completed his residency at the University of Pittsburgh - Shadyside Hospital and is board-certified in Family Medicine. Before joining CHCB in 2011, he had worked for the Indian Health Service on the Hopi Reservation and as a family physician in a community health center in Flagstaff, Arizona. Dr. Saal is currently pursuing a master's degree in public health from Loma Linda University. His professional interests include infectious diseases, health care disparities, and cross-cultural issues in medicine. He and his wife have two daughters and enjoy gardening and raising chickens.
Andrea Solomon, PA-C is a VT native who was raised in this community. In 2003, Andrea graduated from Skidmore College with a concentration in Women's Studies. She completed her Physician Assistant training at the University of New England. Before going to PA school, Andrea worked at the VT Respite House as a House Manager and then as a medical technician at a pediatric ophthalmologic office in NYC. Her medical interests include, but are not limited to, geriatrics and women's health. During her spare time, she enjoys traveling, gardening, and spending time with family and friends
Heather Stein, MD is a native of West Virginia and grew up in Morgantown where her father still practices Family Medicine. Dr. Stein graduated from Virginia Tech with a B.S. in biochemistry and a B.A. in classical studies. She returned to West Virginia University for medical school and moved to Vermont for her Family Medicine residency at Fletcher Allen where she was introduced to the CHCB community. Dr. Stein's professional interests include adolescent/young adult medicine, infectious disease, and care of the underserved as well as comprehensive care for the whole family. She has a wonderful husband whom she met in medical school and who is also a family doctor in the area. She enjoys cooking, gardening and spending time with her furry children (two cats and two dogs).
Abigail Torbert, PA-C joins CHCB from Denver, CO where she worked for the past four years for Stout Street Clinic with Colorado Coalition for the Homeless. She grew up and attended college and PA school in Oklahoma. Underserved populations are her passion and she is excited to continue that work with us here in Vermont.
Wayne Warnken, MD graduated from Western State College and from the University of Colorado Health Sciences Medical School. He completed his residency at the University of Washington Family Medicine Residency-Valley Medical Center. Before medical school Dr. Warnken worked at Samaritan House-Homeless Shelter, in Denver Colorado; for the United States Park Service; and as a research assistant at the Salk Institute. Dr. Warnken's professional interests are in Spanish, addiction medicine, pediatrics and adolescent medicine, nutrition, and sports medicine. When Dr. Warnken is not at work he is active in H.O. Wheeler PTO, City Market and the YMCA.
Jennifer Willingham, MD received her Bachelor of Arts from Middlebury College in 1995. She received her medical degree from the University of Texas, Health Science Center at San Antonio, in 1999. Dr. Willingham completed her residency at the York Hospital Family Medicine Residency program in 2002 upon which she began practicing here at CHCB. Dr. Willingham's professional interests are women's and children's health. In her free time she can be found hiking, knitting or making jewelry.
Dental
Susan S Camp, RDH was raised in Central Vermont and practiced there until 1998. She came to Burlington to work at Associates in Periodontics and then joined CHCB in 2004. Susan is a graduate of Fones School of Dental Hygiene in Connecticut. She is a member of the ADHA, VDHA, and past member of the Vermont Board of Dental examiners and Northeast Regional Board. In 2008, Susan received the VDHA Outstanding Dental Hygienist Award. Outside of CHCB, Susan is a member of Addison County CERT and the Emergency Management Team for the town of Monkton. She is married and has one son, a daughter-in-law, two grandsons, a Rat Terrier, and two cats. In her spare time, Susan enjoys kayaking and reading.
Deepak Devarajan, DMD graduated from Purdue University with a BS in medical technology and subsequently from the University Of Pittsburgh School Of Dental Medicine in 2007. He completed his general practice residency training at Fletcher Allen Health Care in June of 2008. Dr. Devarajan works at the Riverside clinic, the Safe Harbor clinic and also at the H.O. Wheeler elementary school. His professional interests include oral surgery, endodontics, pediatric and family dentistry. Deepak also enjoys traveling and learning about different cultures and communities.
Jennifer Frank Gamage, CDA, EFDA is a native Vermonter and completed the Dental Assisting Program at Essex Vocational Center in 1993. She started working shortly after at The Dental Center moved to CHCB in 2002. She completed the Expanded Functions Dental Assisting (EFDA) Program at the Essex Technical School. The EFDA qualifications allow her to place all fillings, Sealants, Composites and Amalgam. She currently lives in Colchester with her husband and two cats.
Brittney Gendreau, RDH graduated with honors from Vermont Technical College in May 2010 and is in the process of completing her B.A. in Dental Hygiene at Vermont Technical College. When Brittney's not working at CHCB's Main site, she enjoys tennis, kickboxing, hiking and cooking.
Kristina Gilbert, RDH has been a member of the CHCB dental team since 2005. Throughout college Kristina worked as a certified dental assistant. In 2009, Kristina graduated with honors from Vermont Technical College Dental hygiene school and has since been working as a Dental Hygienist. Kristina enjoys outdoor activities such as fishing, snowmobiling, and hiking.
Raba Hebib, EFDA moved to Vermont from Bosnia and Hecegovina in 1994. Raba completed the Dental Assisting Program in 2003 at the Center for Technology in Essex, VT. She completed the Expended Function Dental Assistant Program at the Center for Technology in 2009. EFDA qualifications allow her to place all Fillings, Sealants, Composites, Amalgam and assist in various dental procedures. She was awarded Dental Assistant of the year in 2007. Raba currently lives in Essex with her husband Halid and two daughters, Selma and Alma. Raba has been with CHCB since 2004.
Kevin Risko, DMD has been with CHCB’s Dental Center since 1997 as a staff dentist. Dr. Risko is also the Director of the dental residency program at Fletcher Allen Health Care and a Public Health Dentist at the Vermont Department of Health. He lives with his wife Tami and dog Madison in South Burlington and enjoys reading and traveling. Dr. Risko feels passionate about educating the public about proper oral care and has enjoyed his years as a dental professional.
Rachel Roeloffs, RDH graduated from Vermont Technical College in 2006 with an associate's degree in Dental Hygiene. She grew up in Weybridge Vermont and practiced in Middlebury before coming to the CHCB. Some of her interests outside of working at CHCB include snowboarding, horseback riding, traveling, hiking, golfing, and spending time with her family and friends.
B. Michael Thornton, DMD is a Vermonter and graduate of the dental residency program with over 30 years of private practice experience in Vermont. He is a Fellow of the Academy of General Dentistry. He has been married to his wife, Irene, for over 30 years and is dad to two adopted children, many international students and a wonderful dog named Adidas. He has been active in his church and community and has been with the dental center at CHCB since 2003.
Gopichand Vallabhaneni, DMD moved to Vermont from Portland, Maine with his wife and children where he worked in a private office for five years. Dr. Vall graduated from Boston’s Tuft’s School of Dental Medicine in 1998. Dr. Vall does an array of dentistry and in his free time enjoys playing golf.
Chasity E. Wasco, RDH received her Associates Degree in Dental Hygiene from Broome Community College in Binghamton, NY. She is a member of the ADHA and VDHA. Before joining the dental center at CHCB, Chasity spent several years in private practice in New Hampshire and Vermont. Chasity now spends most of her time at the school-based dental clinic at the H.O. Wheeler Elementary School. In her daily practice, she stresses patient education and preventative care with a smile. Outside of the clinic, Chasity enjoys ballroom dancing, painting, and gardening and can often be found volunteering with the Franklin County Humane Society. She lives in Georgia with her husband, Michael, and their four dogs.
Joe Zuaro, DDS came to the Health Center after 30 years of practice. Dr.Zuaro started his practice in the Public Health Service, stationed in Montana, and then in Island Pond, VT. He moved to Northfield, VT in 1981 where he worked in private practice and retired in 2007. Now Dr. Zuaro is happy to work part-time at CHCB and is the lead instructor of the Expanded Function Dental Assistant course at the Center for Technology in Essex. Dr. Zuaro is also on the Board of Directors for the Vermont Foodbank.
Mental Health
Erin Ahearn, LICSW, LADC, received her B.A. from Saint Michael's College and M.S.W. from the University of Vermont. Erin has been with CHCB since 2007 and is the Lead Clinical Social Worker at the Safe Harbor Health Center, working with clients who are homeless or marginally housed. The focus of her practice is helping people with depression, anxiety, and co-occurring disorders using cognitive behavioral and insight oriented therapies.
Doug Bugbee, LICSW, LADC, received a B.A. from Hamilton College and an M.S.W from the University of Maryland at Baltimore. Doug has worked at CHCB since 2002. He currently facilitates the Recovery Group for Patients struggling with co-occurring addiction and depression or anxiety as well as a Pain Management Group fro those struggling with chronic pain and depression/anxiety. As a clinical social worker with over 20 years of experience, Doug uses a biopsychosocial approach with a perspective that draws on the mind-body connection and the importance of the client's story. He provides Mindfulness-Based Stress Reduction and Cognitive-Behavioral Therapy with a focus on solutions and possibilities for change. He is competent in addressing depression, anxiety, substance abuse, chronic pain, relationship issues, grief and loss, trauma, anger, and compulsive behaviors. He enjoys collaborating with the primary care providers, nurses, and other personnel at CHCB.
Sara Chase, LICSW, LADC, ACSW, received her B.A. in Social Work and Psychology from Trinity College of Vermont and her M.S.W from Florida State University. Sara also attended the City University of New York's Doctoral Program in Social Welfare, which specialized in macro level practice including social policy analysis and social welfare administration. Sara has worked primarily in community mental health as a psychotherapist and social work supervisor specializing in adults with co-occurring disorders. Sara's practice areas of specialization include Cognitive-Behavioral Therapy and Substance Abuse Counseling for substance-related disorders, mood disorders, anxiety disorders, sleep disorders, adjustment disorders, personality disorders as well as trauma treatment, smoking cessation, chronic pain management, stress reduction, mindfulness, and the facilitation of clinical anxiety and stress reduction groups. She has also facilited clinical stress reduction groups for various refugee groups, including: Bosnian, Somalian, and Iraqi. Sara also enjoys providing field instruction for the University of Vermont's M.S.W. and B.S.W. programs and has served several terms on the Board of Directors of the National Association of Social Workers.
Leighton Johnson, LICSW, LADC, received a B.A. from Hobart College and M.S.W. from the University of North Carolina at Chapel Hill School of Social Work. Before coming to CHCB Leighton worked in North Carolina on an Assertive Community Treatment Team with Individuals who had severe and persistent mental illness and who were often homeless and struggling with substance abuse issues. Leighton has been with CHCB since 2010 and works at the Safe Harbor Health Center with individuals who are homeless and in transitional housing. Leighton provides social work coverage for Safe Harbor's medical clinic and provides case management for individuals in the Housing First Program. Leighton provides individual therapy with a focus on treating depression, anxiety and substance abuse issues using cognitive behavioral and motivational interviewing techniques.
Suzanne Potente received her B.A. from the University of Wisconsin-Milwaukee and an M.S.W. from New York University. Suzanne is the OB Clinical Social Worker and assists women throughout their pregnancy. She also provides mental health and substance abuse counseling to patients with a focus on anxiety, depression and trauma treatment. Suzanne has been with CHCB since.
Naya Pyskacek, LICSW, LADC received a B.A. from Sarah Lawrence College, an M.A. from New York University, and an M.S.W. from the University of Vermont. Naya is the Behavioral Health Services Manager and has been at CHCB since 2000. She facilitates Smoking Cessation Groups and is trained in EMDR - Eye Movement Desensitization and Reprocessing (Level II trained) and has served as a field instructor with the UVM Social Work Department for 10 years. Naya's practice focus at CHCB includes: depression and anxiety management, substance abuse counseling, stress reduction, trauma treatment, chronic pain management, grief counseling, and health and behavior change.
Katherine Ruggiero, LICSW, earned her B.A. from Wells College and M.S.S. from Bryn Mawr College Graduate School of Social Work and Social Research. Katherine has been at CHCB since 2010. Katherine's practice focus includes: depression and anxiety management, substance abuse counseling, eating disorders, trauma treatment, and sexual orientation counseling.
Karen Schumacher, LICSW, LADC, earned a B.S. from the University of Vermont and M.S.W. from the State University of New York at Albany, Karen has worked at CHCB since 2009, facilitates a Cognitive Behavioral Therapy group for insomnia, and her practice focus includes: depression and anxiety management, substance abuse counseling, stress reduction, trauma treatment, and health and behavior change.
Suzanne "Suzi" Tanner, LICSW, ASAC, received her B.A. from the University of Massachusetts and her M.S.W. from the University of California at Berkeley, School of Social Welfare. Suzi has been with CHCB since 2009 and provides services at the Pearl Street Youth Center as well as at the CHCB's main site. Suzi's area of specialty includes working with transition age youth, trauma and PTSD treatment, working with survivors of sexual violence, treatment of depression and anxiety, and use of mindfulness techniques.
Psychiatry
Joe Lasek, MD graduated with a BS from Cornell University and received his medical degree from SUNY Upstate Medical University. Dr. Lasek has worked as Associate Medical Director and staff psychiatrist at the Howard Center and now serves as Clinical Assistant Professor of Psychiatry at UVM College of Medicine and Adjunct Faculty at the Southern New Hampshire University. He also has worked for the Vermont Department of Corrections. Dr. Lasek’s professional interests include psychiatric and substance abuse co-occurring disorders, mindfulness & cognitive behavioral therapies, adult ADHD, and sleep disorders, among others. In his free time he can be found reading, watching movies, playing basketball, biking, or hiking.
Patricia Manion, PNP received a BS from the University of Missouri and her Registered Nurses Degree from Lutheran Medical Center. In 2003 she received her M.S. in Nursing from Russell Sage College. Before coming to CHCB, Pat worked for ten years at an inpatient chemical dependency center before moving to a community mental health center in central Vermont for 15 years. Pat is comfortable treating a variety of mental health issues and is particularly interested in helping to make these services readily available to those seeking care. Outside of work, Pat can be found enjoying the outdoors, hanging out with her dog, reading, cooking, and spending time with friends.
