CHC offers Medication Assisted Treatment (MAT) for opiate addiction. MAT uses buprenorphine (Suboxone) together with education, Mental Health/Counseling and other support needed to manage addiction for a lifetime. Medication-assisted treatment for opiate addiction is much like treatment for other chronic illnesses.
CHC is a part of Vermont’s Care Alliance which delivers ongoing care services for opiate addiction in a model called the “Hub and Spoke.” CHC provides Office-Based Opiate Treatment and is known as a “Spoke” within this model. Hub and Spoke services ensure that each patient undergoing treatment has a medical home, a medical provider, a pharmacy home, and access to nurses and clinicians. At CHC, our MAT team includes a Nurse and Mental Health Clinical Care Coordinator to provide support to patients in their recovery, and is a free service to Medication Assisted Treatment patients. We help lift barriers to care and provide the support patients with complex addictions need to be successful in their recovery.
