As an integral part of the health care management at CHCB, the Behavioral Health staff is here to help patients of all ages with a broad range of mental health conditions. We employ Licensed Clinical Social Workers who provide therapy and consultation generated by internal referral from our medical staff. Our Behavioral Health clinicians also work collaboratively with our medical providers and nursing staff to support patients who are overcoming addictive illness, including a Medication Assisted Treatment Program for opiate addiction. If you are interested in counseling at CHCB, let your medical provider know at your next visit.
If you or a loved one is experiencing a crisis and cannot reach your CHCB provider, please refer to this list of local support lines.
Behavioral Health Consultants
The Behavioral Health staff is available to the medical providers and patients for consultation during the medical visit. A Clinical Social Worker can help assess your mental health condition or addictive illness. We can also provide education and self-management skills training to manage a mental health condition or to alleviate stress.
Medical Case Management
Social Work Case Managers are also available to help with a variety of social services, such as housing assistance, applying for benefits, or help with care coordination.
