At this time, there is a nationwide shortage of Dental Hygienists which has greatly affected CHC’s ability to schedule dental cleaning appointments. We understand this may be inconvenient and we apologize if this has impacted your care. Thank you for your patience as we navigate staffing shortages.
CHC’s Dental Center is the only Federally Qualified Health Center Dental Program serving Chittenden County and southern Grand Isle County. With dentists and a large support staff, we care for all ages at our Riverside, Champlain Islands, South End and Winooski locations and a School-Based Dental Center to offer a full complement of preventive and restorative care.
Dental services are available on our Sliding-Fee Scale to income-eligible, and/or uninsured residents. We welcome Medicaid and private employer-based insurance for adults and children. For low-income, uninsured community residents with dental emergencies, we also accept state general assistance vouchers.
Dental Patient Medical History Form
Completed Registration and Dental Patient Medical History Forms can be returned to us in-person, by mail, via e-mail at dentaltriage@chcb.org or sent by fax to (802) 652-1056. Once we have received your completed forms, we will call you to schedule an appointment. We are currently experiencing a large volume of people seeking dental care, and are doing our best to accommodate the increase in appointment requests.
Please note: If you have had any dental X-rays within the last three years, please have them sent to us as well. This will enable us to receive the information we need in a timely fashion and best schedule to your needs.

Appointment Cancellation
Due to limited availability for Dental appointments, we have strict rules around no shows and late cancellations for adult patients (18+). Patients who are unable to make an appointment should call us at least 24 hours before their scheduled appointment so we can use the time to see other patients.
