The Community Health Center of Burlington accepts all patients regardless or their insurance or financial status. Our federally funded health care program allows us to offer a wide range of services to patients through a sliding fee scale; we also take most employment-based insurances.

For dental services, we also offer a sliding fee scale to all income eligible dental patients that live or work in Chittenden County or patients coming from Addison, Lamoille, or Grand Isle counties that require urgent care. The income guidelines and acceptable verification for the slide are listed below. To obtain more information please contact our Patient Accounts department at (802)864-6309 ext 156.

- Most recent Federal tax return
- IRS form W-2 or 1099
- 2 recent consecutive paystubs
- Social Security, disability or pension benefit statements
- Documentation of other governmental assistance
- Verification of Student status and FAFSA form

For uninsured medical patients who are not eligible for our sliding fee scale, we offer a Time of Service Discount. This discount will be applied on all charges paid for on the date that the services are provided and can help reduce the cost of your visit.
Many of our patients are also eligible for Vermont’s health and dental care programs. These State and Federally funded programs offer affordable coverage for qualified Vermont residents and their families. The Community Health Center of Burlington accepts all forms of these health plans.
