The Community Health Centers of Burlington accept all patients regardless or their insurance or financial status. Our federally funded health care program allows us to offer a wide range of services to patients through a sliding-fee scale; we also take most employment-based insurances.
Eligibility
The Community Health Centers of Burlington offer a sliding fee scale to all income eligible patients based on annual household income for all of our services. For dental services, we also offer a sliding-fee scale to all income eligible dental patients that live or work in Chittenden County or the towns of North Hero, South Hero or Grand Isle. Patients, who are income eligible, coming from Addison, Lamoille, or Grand Isle counties can usethe Sliding-Fee Scale for urgent care dental care. IMPORTANT NOTICE: Sliding-Fee Scale Changes effective June 1, 2013

Acceptable forms of Income Verification:
- Most recent Federal tax return
- IRS form W-2 or 1099
- 2 recent consecutive paystubs
- Social Security, disability or pension benefit statements
- Documentation of other governmental assistance
- Verification of Student status and FAFSA form
- Unemployment Benefits
- Patients with no income will need to speak with a patient account representative
For uninsured medical patients who are not eligible for our sliding fee scale, we offer a Time of Service Discount. This discount will be applied on all charges paid for on the date that the services are provided and can help reduce the cost of your visit.
Many of our patients are also eligible for Vermont’s health and dental care programs. These State and Federally funded programs offer affordable coverage for qualified Vermont residents and their families.
