Can't remember the last time you saw a doctor? Worried about your health? Feel like you can't afford health care?
The Community Health Center of Burlington (CHCB) in partnership with Fletcher Allen Health Care, would like to take care of you on Sunday, March 14, 8 am - Noon.
Call 864-6309 (press option 1) by March 12 to schedule your Free Clinic Day appointment. First call, first served, up to 100 appointments.
At no cost, you'll receive an office visit with a medical provider, blood pressure/glucose screening, help with applications for state health insurance and prescription drug programs, introduction to the services provided at CHCB, help accessing the regular, on-going health care needed and assistance scheduling any needed follow-up appointments. We are not able to schedule Free Clinic Day appointments for chronic pain or addiction.
CHCB is Chittenden County's Federally Qualified Health Center. We provide affordable health and dental care for all ages. Our services include primary and preventative health care; dental care; mental health and substance counseling; obstetrical, prenatal and well baby care; mail order pharmacy and low cost prescription assistance programs; and assistance with applications for public health insurance programs.
CHCB accepts employer based health and dental insurances, Medicaid and Medicare and offers a sliding fee scale for uninsured, income eligible patients.
